Post op wound care advice!!

Hi All,

Am so glad I found this site, an absolute wealth of info!

I'm after some advice please. I had my sinus excised 2 weeks ago tomorrow and was left with an open wound about 1.5" long, 1/2" wide and 1" deep that starts about 1cm from my butt hole.

Following the op it was packed with Alginate tape for a the first few days and then the 2nd week the nurse said it only need to be filled with Intrasite gel and topped with guaze to simply keep it in place.

After my last visit to the nurse she now wants me to leave it with nothing in and uncovered for some of the time to let it dry a little

I shower after every BM which are very often as my eating is all over the place and bowel habits (4x perday rather than once or twice and very small amounts) have gone bonkers compared with normal. I think this could be down to the worry of having to go and the hassle of very careful delicate wiping, shower and then getting my Fiance to sort the dressing.....

So my question is this.... when its left totally undressed Im worried that even farting will cause bacteria to get into the wound and cause an infection. Is this something I should be worried about???

Secondly, I never seem to feel like I have totally emptied my bowel, as soon as I stand up from the loo I feel like I need to go again....????

To me It seems like you have a fistula , probably .Did your surgeon conformed that what you have is a pilonidal ? According to the measurement it is a very small incision . by the way I used to have a fistula and I had a surgery for it , it works great no more issue .Im not a Doctor , but just my experience .due to the area the incision need to be clean to avoid any kind of infection , for the Hygiene I used to use a warm sitz bath and dray the area gently with a soft towel No toilet paper , I wouldn't worry about farting
about you dont feel like you empty your bowel , that could be a sign of IBS , you should adjust your diet add more fiber eat lot vege. fruits and drink plenty of water . but my best advice to you is you need to see a very experienced colon rectal surgeon for help . I hope you can benefit from my experience

Thanks for the quick response!! My surgeon was concerned pre op that it may have been a fistula due to location but following surgery he explained that it was only a blind sinus which was excised. Rest assured he is very experienced in this area.

Any other advice gladly recieved!

I'm not sure how toxic your farts are, but I think you're pretty safe. Hopefully one of the physicians on the forum can contest to that.

As long as the wound has exudate.. any small foreign particles should be discharged in that... like dust particles. You just don't want bacteria in there.... (mainly from your hands or non-sterile instruments)

The alginate and other packing should be keeping the wound open and discharging. 1" deep is a fair bit of tissue to rebuild... months. Has the nurse told you what the plan is? They will be keeping it moist enough not to scab but dry enough not to macerate... I don't know how they do it, but that is the nurse's skill. But I'd be wanting to understand if the gel has made it too moist, how long exactly it should be exposed per 24 hrs, and especially why it's not being packed and if you're working back to that?? Or f we're expecting the flesh to just rise??

Mine has been packed daily and painstakingly by nurses for months until the packing is that small you can hardly grab it with your fingertips the opening less than 0.25"... I'm expecting them to stop any day... there ill still be some healing to complete...

You could have completely different circumstances and others might have experience with that intrasite gel. I am no expert sorry... just personal experience
