I'd look at the hall of fame surgeon list, and see which surgeon is closest to you, or maybe considering travelling to see an expert. My scar split in January, I went to see GP, Nurse and got advice from tissue viability, and all 3 have said it looks like a simple surface wound, 7 weeks on and that "simple surface wound" is still open, two weeks today I will see a pilonidal expert, if she says its a simple surface wound, then I'll carry on trying to heal this simple surface wound, but knowing my history it will be more than that, what I'm trying to say is you need to get an experts opinion on it, because in my opinion, the doctors/nurses close to me know sweet f*** all.
