Hello all, I'm new here so I'll give some info about past medical history and my experience with PSD. I am a 19 year old female and had my first pilonidal abscess when I was 18. I had no idea what it was but I remember feeling nauseous, not eating, having night sweats/low grade fever along with my first infection. I saw my GP the first time and she saw no visible bumps so she thought it was just inflammation. That night the abscess ended up bursting (which at this point I was really concerned what was going on) so I went back to my GP and she diagnosed me with a pilonidal abscess and prescribed me a multitude of antibiotics. It was VERY challenging to take the antibiotics so I went to the ER and after having them take a look I ended up having not only one but two abscesses. So I had the lancing procedure and was on meds for a week and then felt a lot better. Surgery was kinda discussed but he seemed hesitant on whether it would come back?

Fast forward a year and a half later, literally a day after my last final exam at uni my cyst ended up infected again. The infection hasn't been nearly as intense, I'm eating fine and there is no sickness. I saw my GP and she prescribed me amoxicillin augmentin (I am on day 4/10 two tablets a day). I've been taking hot showers at least 3 times a day to keep the area clean and I've been trying to keep the area as dry as possible. The abscess ended up bursting again and I feel a TREMENDOUS amount of relief. Is any kind of lancing procedure necessary still? I plan to keep the area as clean as possible but it is draining a lot so I just have a menstrual pad on. Will the antibiotics clear up the infection? I plan to schedule a consultation with my surgeon to see about getting surgery to have the cyst removed. I was hoping someone could tell me more about my surgical options!

These are all your options with most surgeons opting for open/closed excisions which on here you will find most people have failed ones. These can work, but the recovery to me seems worse than some of the recommended surgeries which are cleft lift, pit picking, epsit.

I don't have any surgical info for you (I haven't opted for any surgeries yet) but I can answer a few of your other questions.

It's good that your abscess burst, and to your question if additional lancing needs to be done...maybe. I had an abscess that had burst but did not fully drain - i had it lanced after that and it was amazing what still came out of it. Apart from the lancing, the doc also squeezes and packs it, much more thorough than what the wound will do on its own. It just seems like the more thorough they get, the better the results. To your other question about antibiotics, yes the antibiotics will resolve the infection, but lancing is still good in order to remove the biological material (pus, blood, buildup, etc) that's generated by the wound. It's a two-pronged approach that seems to work the most thoroughly to get you through the abscess.

But as you are probably understanding from reading this board, surgery is inevitable and these cysts do not go away, they merely recede into what I call "resting mode" between abscesses.

I'm in the midst of a pregnancy right now hence why I'm not looking into surgery options (too much on my plate!!!) but it's inevitable for me, too.

Put some max strength desitin diaper rash cream over your cyst. Only 40% Zinc oxcide worked for me. And I never agreed to surgery it may help to prevent another pilonidal infection. Had mine for 8 years and this stuff keeps me away from surgery . along with antibacterial soap when showering should help to prevent another infection.

Thank you so much for the surgical information! I just had an open excision today and so far everything has been very well. It's a bit tender but the large abscesses I got were far more painful than the post op pain I have been experiencing.

Interesting on the zinc oxide. I have the max strength but was using it for a fissure. I have an open pit, is it ok if it gets in the pit?
