Hello,

I had surgery for my pilonidal cyst 40 years ago at the age of 18, and I'm now 59 years old. Over the years the area has gotten a bit inflamed and somewhat painful at times, but nothing too serious. Unfortunately, 6 months ago I began having pain in the area of my old scar and sitting started to be painful. I realized that a small bump had formed within the scar. It took me a while to get a referral through my HMO, but I finally got an appointment with a colorectal specialist, after waiting for several months. I finally got to my appointment last month, only to be told the dr was called into surgery, and he couldn't see me! I ended up getting examined by the PA, and she said for my case they could do a trephination procedure (I think it is also called Gips). I wasn't able to get another appointment to see the actual surgeon for another month, which is tomorrow.

Has anyone on here had the Gips procedure? If so, what was your experience like, and have you had any reoccurrences of the pilonidal? The PA made it sound very non-invasive and easy to heal from. My main concern is if it will be as effective as the other types of surgeries.

I think it’s reasonable to be cautious, especially since this is an old surgical scar and the symptoms have appeared again after 40 years. The Gips/trephination procedure does sound appealing because it is much less invasive, usually involves small openings rather than a large incision, and can mean a quicker recovery and less time away from normal activities.

This procedure is usually performed quickly under local anesthesia. The doctor will use a small surgical instrument (trephine) to precisely remove the fistula (pit), suctioning out pus, inflamed tissue, and accumulated hair underneath without making a large incision. The resulting wounds are just small, round holes a few millimeters in size, left open to heal naturally from the inside out. Patients rarely need strong painkillers and typically only need 1 to 2 days to return to normal activities and mobility. Changing dressings is also simpler and much less painful than with conventional open surgery
